Chiropractic and Angina
Since Dr. Jane and I have been in practice, we have noticed a lot of success in treating people with chest pain that seem to have normal EKG's, stress tests and angiograms. Now, there is broad agreement that the musculoskeletal system is a potential source of pain in non-cardiac chest discomfort.
Chest pain can arise from both cardiac and non-cardiac disease. The term angina pectoris (AP) usually refers to the cardiac causes of chest pain thought to be due to myocardial ischemia (oxygen deprivation to heart muscle). Non-cardiac sources of chest discomfort may account for up to 50% of cases that present to a cardiology ward. The differential diagnoses can include pulmonary, gastrointestinal, psychosocial and musculoskeletal disorders. Non-cardiac sources of chest pain usually have an excellent prognosis for survival and rates of cardiac disease similar to the general population. However, disability and residual chest pain are common in this group. Less than half of patients are reassured that they do not have serious pathology. Despite the fact that not many of these patients have identifiable heart problems, many remain on cardiac medication.
Before a musculoskeletal issue is pursued in relation to angina is important to rule out cardiac problems first. But, patients with angina pectoris and normal coronary angiograms have been shown to have a high frequency of musculoskeletal pathology and dysfunction (see here). Positive findings peaked at levels T1–T6 (physical examination) and at levels C4–C7.
Recent evidence supports our experience that patients with known or suspected angina pectoris may benefit from chiropractic.
